Secure Patient Intake Forms

The Health Insurance Portability and Accountability Act (HIPAA) that was introduced in 1996 established strict and specfic regulations regarding the transmission and storage of Protected Health Information.

Email, unfortunately, is a very unsecure way of transfering information and does not adhere to HIPAA regulations.

It is illegal to send Protected Health Information such as Patient Intake Forms, X-Rays and Referral Slips via email.

Our system solves this problem by encrypting all data sent and stored with our secure forms system, thus ensuring that you are fully HIPAA compliant.

How it Works

  • To start, the patient creates an account with simply an email address and password.

  • The forms are then available to fill out on your website.

  • If he doesn't have all the information to complete the form at that time, all is information is saved for a later time.

  • Once filled out, you're notified via email and fax.

  • You then can log in and download the form in a printable format.
